2021 Membership Application

This 2021 membership form can be printed and mailed in to the address at the bottom of the form. Once it is filled out completely, you can send it along with full payment.

If you are printing this out due to not receiving a ballot for board members elections, you can vote by printing out the ballot also and mailing it along with the membership form and payment.

There can only be 1 membership,1 payment and 1 ballot in an envelope.

Your return address must be on the envelope. All entries will need to be received by Dana by Jan 22, 2021.

Votes will be tallied that evening. No late entries will be accepted. The ballot will be posted on the website by Jan 9th. All nominations need to be done by Jan 8th and ballots will be sent out on the 9th.Dana is receiving nominations by phone call or text to 515-341-9034 or email iowaharnesshorseassociation@gmail.com.
